You can easily apply for the Preferred Care program by downloading and completing the application and send it to PGH via fax, e-mail or post. Please keep in mind that residents of the USA (those who reside in the USA for more than 183 days per year) and applicants over the age of 75 are not eligible for the Preferred Care program.
To Apply:

- Print pages 1 and 2 of the PGH application form.
- Fill out the application and either mail, fax or e-mail to:
